Band Registration
All bands participating in the U.S. Open must complete and submit the following. All forms are included in the PDF file below.
Please note: You must save the file to your desktop before filling out the form to ensure the entered data is not lost
- Band Registration Form and $250.00 Registration Fee - Due by July 12, 2008
- Program Requirements - Due by October 4, 2008
- Band roster
- Band history and photo
- Conductor bio and photo
- Contest program
- Announcer notes
- Percussion needs
- Please note any piece to be considered for a special award
(Best Performance of a March, Best Solo, Best New Composition or Arrangement) - Member Registration Form and Payments - Due by October 4, 2008
- Contest admission tickets for all band members
- Contest admission tickets for guests (family members, friends, etc.)
- Banquet admission for all members and guests who plan on attending - Scores of Contest Program - Due by October 4, 2008
- Music scores (one original and two copies for each piece)
- Buskers Registration Form - Due by October 4, 2008
